Incoming information from the patient is summarized in highly customizable patient reports. Each element of these reports can indicate changes in function, symptom, health trend or other issue. Every item is traceable along the timeline and supports the medical teams with different analysis tools for the decision-making process.
Every element is also actionable: e.g. ordering additional patient reported data, sending the patient targeted educational materials and additional advice from the care team. This should support the patient in disease management outside of the clinic.

Learning and advising

Patient Monitoring and Education (PE) provide an excellent opportunity for the care and support of patients outside the medical facility.

However, they are also associated with additional personnel resources. To reduce this problem, meQ Ai Care analyzes the data received from the patient and the actions taken by the medical team. From this analysis, the system generates suggestions for process optimization and possible automation. Such automated performance of risk-free actions provides excellent opportunities for integrating PROM and PE into the routine, with no additional burden on medical staff.

Automatization

Certain outgoing information, like PROM instruments and supportive medical content, can be automated. Ai Care delivers reports and suggestions about which parts of the process can be automated.

By the way, these actions can also be reminders, instructions, checklists, preparation material and much more. And of course, all of these actions are linked to feedback reports, so medical teams always keep track of status.

Alerting

Of course, there always will be certain situations related to critical care or unknown changes in the monitoring process of the patients which require direct interaction and handling by the medical team to prevent a deterioration of the patient’s condition.

For those cases Ai Care delivers alerts to the responsible medical teams, using different triggers like Cut-Offs, Cross item relations or just recognizing unknown conditions and unexpected changes. Those alerts can be sent as service order (via standardized protocol e.g. HL7/GDT) to different third-party systems (e.g. Hospital information systems and Praxis software).

WiDB

All validated PRO-instruments, supporting information and informative articles are located in the knowledge base (WiDB) of Ai Care . Additional information or [actions] can easily be added at any time. If the Adviser doesn’t have a pre-entered solution for a particular indication or problem, it can be created by the medical team as an extended note.

These manual entries can be analyzed and entered into the WiDB as a standardized solution after validation.  In addition, Ai Care’s automated reports, statistics and visual dependencies help edit and deliver updated  content to the database.
